Registration Guidelines

Online registration is closed.

Please read the instructions on the website carefully.

Upon submitting the Registration Form, each participant will receive a PRN (Participant Registration Number) by e-mail to be included in payment details (in case of payment by bank transfer). Please do not send any payments (bank transfers) without PRN included in payment details. Such payments cannot be identified. All participants are kindly requested to register in advance by completing and submitting the Registration Form with the appropriate fees.

Registration Fees

Please note that unpaid pre-registration will not be considered as a valid registration unless fully paid and confirmed. Pre-registration summary therefore cannot be taken as a valid registration confirmation. Please make sure to pay your registration fee within the time frame below to benefit from the bonus discount rate or early bird fee registration.

If your payment is not received before deadline date the later fee will be automatically charged.
 

Because of limited space available in the main plenary hall at the Prague Congress Centre, the fee (39 €) for the live opening ceremony and the welcome reception has been separated from the full Congress fee to ensure the fair allocation of seats on a first come basis. Only delegates attending the full Congress are eligible for opening day tickets in the main plenary hall which will be allocated on a first come basis. The Opening Ceremony will be relayed to an overflow area in the Congress centre (the Forum Hall) when all seats for the plenary hall have been allocated. The fee for the relayed ceremony and the welcome reception is 20 €. The website will indicate when all plenary hall seats have been allocated at the time of booking online.

Registration after 9 May 2014

Online and postal/fax applications will be closed from this date. Delegates wishing to register after 9 May 2014 should visit the Congress website, download the registration form, and bring the completed form and fee to the onsite registration desk at the Prague Congress Centre. A receipt will then be issued.

Terms and Conditions

All fees include VAT. If your payment is not received before the deadlines for Bonus discount 30 June 2013 and Early Bird rate 21 February 2014 the later fee will be automatically charged. Pre-registrations will no longer be accepted by the Congress Secretariat after 9 May 2014, but you will be able to register on-site from 31 May 2014. For group bookings, please contact Congress Secretariat.

* A student midwife is defined as person pursuing a recognized programme of study leading to a qualification as a midwife, approved by the regulatory authority in the country, in line with the ICM definition of a midwife. The name of the training institution or college attended is required with the stage of study (eg. 2nd year). Proof of student midwife status will be required at the time of registration.

** Qualifying period is up to 5 years after qualification at the time of registration. Payment and a copy of the certification document including date of qualification will need to be provided at the time of registration.

*** Free registration as part of the reserved exhibition space. Two registrations per 9sqm of the exhibition stand area. Exact booth number and company must be stated in the remark during the registration. All registrations must be approved by exhibition agent.

**** Additional registration for the exhibition booth personnel. Exact booth number and company must be stated in the remark during the registration process otherwise the registration will be canceled.
 

Insurance

The registration fees do not include insurance of participants against accidents, sickness, cancellation, theft, property damage or loss. Participants are advised to take out adequate personal insurance.

Only one Accompanying person per registered delegate is allowed.

All Participants need to re-confirm attendance of all functions in their Registration Form.

Confirmation

Your registration will be confirmed by Congress Secretariat once your registration and payment have been received. Your confirmation will be forwarded on to you within 21 days. This confirmation letter must be presented upon arrival at the pre-registration desk if the account has been settled, or at the non-paid desk if a balance is still due. If you do not have a confirmation letter, please bring along your passport or other form of identification.

Please note that registration for the various tours and events will be confirmed upon payment on a “first come, first served” basis and numbers may be restricted.

Payment

All fees for the Congress registration should be paid in EUR (€), free of all bank charges!

Bank transfer

Clear of any bank charges, clearly stating the participant’s name and PRN (Participant Registration Number)
Bank transfer can be made only upon receiving detailed information (account number, PRN etc.) from the Congress Secretariat. Payments received without pre-registration details (name, PRN) cannot be identified.

Online payment by Card (Visa, Eurocard/Mastercard)

In case of rejected on-line credit card payment, the registration will not be processed by the Congress Secretariat. It is recommended that the participant contacts credit card issuer/bank first and checks if the on-line transactions are not disabled on his credit card. Afterwards, the online registration process has to be repeated.

Credit Card Acceptance Policy

Please kindly note that for the purposes of the online registration and payment, only internationally recognized credit cards using highest standards of security are accepted. We accept following credit cards for the online payments: MasterCard, Maestro, VISA, VISA Electron a V Pay. For your easier reference, please check if your credit card is embossed, has 16 digit number and 3 digit CVC code at the back plus a magnetic slip. Should you have any question in regards of validity of your credit card for online registration, or should you wish to check if payments with other cards like American Express are possible, please feel free to contact the Congress secretariat here.

Cancellation / Refund Policy

All cancellations must be notified in writing to the Congress Secretariat according to the conditions below.

Cancellation of registration

  • on or before 4 December 2013 - 75 € administrative fee will be deducted from the refund
  • from 5 December 2013 till 1 March 2014 – reimbursement of 50 % will be made
  • on or after 2 March 2014 - no refund will be made under any circumstances

Cancellation of extras

  • on or before 4 December 2013 - 20 € will be deducted from all refunds
  • from 5 December 2013 till 1 March 2014 - 30 € will be deducted from all refunds
  • on or after 2 March 2014 - no refunds will be made under any circumstances

For any change of name after 11 May 2014 a fee of 50EUR will be charged.

 

Consent to the Processing of Personal Data

This consent is granted for all of the information contained in this form, and throughout the five years from the date of consent. At the same time I am aware of / and of their rights under § 12 and 21 of Act No. 101/2000 Coll. on the protection of personal data.

Summarized data from this form may be used by the administrator for internal statistical purposes and for internal use of the administrator.

Statement by Administrator

Data administrator declares that they will collect personal data to the extent necessary to accomplish the intended purpose and may be processed only in accordance with the purpose for which they were collected. Employees of the administrator or other individuals who process personal data under contract with the administrator and other persons are obliged to maintain the confidentiality of personal data, even after termination of employment or work.

Responsibility

The participant acknowledges that he/she has no right to lodge damage claims against the organisers should the holding of the Congress be hindered or prevented by unexpected political or economic events or generally by force majeure, or should the non-appearance of speakers or other reasons necessitate programme changes. With registration, the participant accepts this proviso.

Liability

For reasons beyond its control the ICM 30th Triennial Congress and/or its Agents, the CZECH-IN s.r.o. and/or its agents have the right to immediately alter or cancel the Congress or any of the arrangements, plans, timetables, sessions or any other items directly or indirectly relating to the 30th ICM Congress. Participants shall not be entitled to return of fees or any compensation for damages that may result from any change, alteration or cancellation. Furthermore, with the exception of any wilful damage or gross negligence committed by the ICM 30th Triennial Congress and/or its agents, the CZECH-IN s.r.o. and/or its agents, neither ICM nor its agents shall at any time be liable for any direct or indirect damage suffered by the participants including consequential and immaterial damage, caused by failure to comply with any provision of this document. It is highly recommended that all participants carry proper travel and health insurance.
